Job Title: Senior Geriatric Care Physician Assistant

Maximize the quality of life for chronically ill elderly members by collaborating with interdisciplinary teams to develop and implement comprehensive Plans of Care.

Key Responsibilities:
  • Conduct thorough health histories and medical exams to determine initial Plans of Care.
  • Initiate, evaluate, and review medical orders to identify opportunities to improve care.
  • Perform various procedures and document all care and recommendations for fellow team members.
  • Order, interpret, and follow up on lab tests and results.
  • Request consultations from in-house and external providers, including physicians, therapists, and specialists.
  • Educate staff, members, caregivers, and families on essential care functions to ensure maximum independence.
  • Complete ongoing assessments to determine Plan of Care changes or improvements based on member feedback and medical evaluations.
