We are seeking a highly skilled and compassionate Patient Navigator to join our team.
- Job Summary: The primary responsibility of this role is to support licensed providers with the management of their patient panels, establishing relationships with patients and introducing them to the patient-centered care team model.
Key Responsibilities:
- To ensure that patients receive timely treatment and are informed of their plan of care, including all follow-up care.
- To establish relationships with patients and introduce them to the patient-centered care team model.
- To scrub and prepare patients' electronic medical records 3 days prior to their visit.
Requirements:
- Education: Graduation from an accredited Medical Assisting school.
- Experience: One year of experience in a related field.
- Skills: Ability to read and interpret documents, write routine reports, and communicate effectively with patients and colleagues.
