Neuro Rehabilitation Social Worker

We are seeking a dedicated and empathetic professional to join our rehabilitation team at a neuro rehabilitation facility. As a member of our interdisciplinary team, you will work in the comfort of home and community settings, providing high-quality care to individuals with neurological conditions.
Responsibilities- Conduct comprehensive assessments to identify the social, emotional, and support needs of patients and their families, and develop individualized care plans that address these needs.
- Work collaboratively with the rehabilitation team to provide direct social work intervention and instruct patients and family members in coping with social and emotional responses connected with the injury or illness.
- Regularly review the effectiveness of treatment plans and progress towards goals, making adjustments as needed.
- Master's Degree in Social Work or Marriage and Family Therapy (LCSW, MSW, MFCC, or LPC).
- Current licensure/certification in the state(s) of practice required by individual states.
- A minimum of one year of experience in a rehabilitation setting serving individuals with neurological conditions.
