Job Title: Case Manager

The position entails working with pediatrics and/or adult populations depending on business need. As a Case Manager, you will work with various healthcare professionals to establish effective discharge plans.

About the Role:
  • Ensure adherence to Case Management Department policies and procedures.
  • Apply Milliman Care Guidelines criteria for appropriateness of transition to care by level of skilled needs and acute rehab for post-discharge plan.
Responsibilities:
  • Coordinate initial and concurrent reviews based on review of medical records received and accurate daily reporting of inpatient admissions.
  • Review medical records and other documentation to determine medical necessity, appropriateness, and level of care utilizing Milliman Care and other relevant guidelines.
  • Assess, plan, implement, coordinate, monitor, and evaluate options and services to reduce hospital readmission rates.
Requirements:
  • Active LVN or RN license in California.
  • Proficiency in Microsoft applications such as Word, Excel, and Outlook.
Additional Requirements
  • Strong communication and interpersonal skills.
  • Ability to prioritize tasks and manage multiple projects simultaneously.
