Job Description:

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team in home care. As an LPN, you will have the opportunity to make a meaningful difference in the lives of others by providing high-quality patient care.

Key Responsibilities:
  • Utilize the nursing process to assess, plan, implement, and evaluate patient care in accordance with established standards and protocols.
  • Provide skilled nursing services to patients in their homes, including medication administration, wound care, and vital sign monitoring.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of each patient.
Requirements:
  • Current licensure as an LPN/LVN in the state where you will practice.
  • One year prior professional nursing experience is preferred.
  • Ability to work independently with minimal supervision and as part of a collaborative team.
