St. Luke's Health System is seeking a Telehealth Patient Specialist to join our Ambulatory Care team. In this role, you will help support patients with chronic conditions. You will be an essential part of a collaborative team focused on improving access to care, reducing hospital visits, and empowering patients to manage their health more effectively.
Onsite role
Position Details:
Shift:3 x12-hour shifts (7:00 AM to 7:00 PM)
Schedule: Rotating weekends and holidays required
Pay Range: $17.32 – $19. plus, per hour, depending on experience
What You'll Do:
Onboard new patients into the remote patient management program
Monitor and manage patients living with chronic conditions
Assist patients in adhering to care plans and maintaining compliance with program guidelines
Provide basic technical support for remote monitoring equipment
Perform scheduling functions, similar to a clinic setting
Utilize medical terminology and clinical knowledge to support communication between patients and care teams
Qualifications:
High school diploma or equivalent required
Preferred Qualifications:- Experience as a Certified Nursing Assistant (CNA), Unit Clerk, or in a similar role- Familiarity with medical terminology- Background in a healthcare setting, ideally ambulatory or telehealth
