As the Care Coordinator, you will be responsible for monitoring and supporting the daily activities of services being provided to clients in accordance with state regulations and company policy, while ensuring quality coordination of service for our clients.At Traditional Home Care we focus on delivering non-medical, personal care to our customers in their home.
Our direct care workers/home health aides help our customers with Activities of Daily Living such as dressing, bathing, grooming, and meal prep so they can continue to live independently in the comforts of their own home.Duties and responsibilities include but not limited to the following:Provide superior customer service and quality home careBuild lasting relationships with clients, referral sources, payers, and community organizationsCoordinate client services and manage scheduling issuesManage and grow your client caseloadDevelop strong, communicative relationships with the teamInterview, hire, and orient reliable and compassionate caregiversFunction as primary support for daily EVV and payroll functionsRequirements/QualificationsPrevious home health care or home care experience highly desirableHigh School or GED required.
Associate's Degree preferredStaffing experience preferredExcellent verbal/written communication, time management and organizational skillsAbility to effectively multi-taskEffective problem solving skillsStrong presentation and training skillsStrong customer service skillsWhy Work at Traditional?
