Nurse Care Coordinator

This esteemed position offers a unique opportunity to make a profound impact in the lives of patients and residents. As a dedicated healthcare professional, you will provide exceptional care for individuals with complex diagnoses, fostering growth and enhancing knowledge and skills.
Responsibilities:- Conduct comprehensive nursing assessments and collaborate with the interdisciplinary team, patients, and families to develop effective plans of care.
- Establish realistic, measurable short- and long-term patient goals, implementing interventions as needed to minimize rehospitalizations and continuously evaluate outcomes.
- Administer medications and perform treatments according to physician orders.
- Delegate patient care responsibilities to staff and supervise them to ensure tasks and interventions are implemented.
- Communicate patient information with assigned staff and between shifts.
- Must be a graduate of an approved nursing program and currently licensed by the state board of nursing.
- CPR certification is required.
This role requires a compassionate and skilled individual who can deliver high-quality patient care while collaborating with the healthcare team.
