- Respond to requests for services and information from patients, families, and other stakeholders.
- Collaborate with therapists, nurses, and psychiatrists to ensure effective care coordination.
- Communicate effectively with patients, families, and healthcare providers to promote positive outcomes.
Requirements:
- Master's degree in Social Work, Counseling, or a related field.
- Licensure as a Registered Nurse, Licensed Social Worker, Licensed Clinical Social Worker, or Licensed Professional Counselor.
- 3-5 years of experience in a behavioral health setting, with 1-2 years of experience in assessment and referral preferred.
