Job Title: Licensed Practical Nurse

We are seeking a compassionate and skilled Licensed Practical Nurse to join our healthcare team.

Overview:

  • Plan, direct and provide care to residents according to physician orders and the Plan of Care.
  • Administer medications and treatments as prescribed by physicians.
  • Perform resident assessments to identify needs and develop effective care plans.
  • Participate in daily resident rounding to ensure high-quality care.
  • Assist with insertion and maintenance of IV therapies for residents as needed.

Responsibilities:

This role is responsible for delivering high-quality patient care and collaborating with other healthcare professionals to achieve optimal outcomes.

Key responsibilities include:

  • Providing direct patient care and support services to residents in long-term care settings.
  • Assessing resident needs and developing individualized care plans to meet those needs.
  • Collaborating with interdisciplinary teams to coordinate care and improve patient outcomes.
  • Maintaining accurate records and reporting any changes or concerns to supervisors.
  • Staying up-to-date on industry developments and best practices through ongoing education and training.

Requirements:

  • Active licensure as a Licensed Practical Nurse (LPN) in Pennsylvania required.
  • At least one year of experience working in geriatric or long-term care settings preferred.
  • CPR certification required.
  • Excellent communication and interpersonal skills essential for effective collaboration and resident engagement.
