Compassionate Patient Care Specialist

We are seeking a compassionate and skilled Licensed Practical Nurse to join our healthcare team.
Overview:
- Plan, direct and provide care to residents according to physician orders and the Plan of Care.
- Administer medications and treatments as prescribed by physicians.
- Perform resident assessments to identify needs and develop effective care plans.
- Participate in daily resident rounding to ensure high-quality care.
- Assist with insertion and maintenance of IV therapies for residents as needed.
Responsibilities:
This role is responsible for delivering high-quality patient care and collaborating with other healthcare professionals to achieve optimal outcomes.
Key responsibilities include:
- Providing direct patient care and support services to residents in long-term care settings.
- Assessing resident needs and developing individualized care plans to meet those needs.
- Collaborating with interdisciplinary teams to coordinate care and improve patient outcomes.
- Maintaining accurate records and reporting any changes or concerns to supervisors.
- Staying up-to-date on industry developments and best practices through ongoing education and training.
Requirements:
- Active licensure as a Licensed Practical Nurse (LPN) in Pennsylvania required.
- At least one year of experience working in geriatric or long-term care settings preferred.
- CPR certification required.
- Excellent communication and interpersonal skills essential for effective collaboration and resident engagement.
