Job Title

We are seeking a dedicated professional to fill the role of Supports Coordinator.

Key Responsibilities
  • Assessment: Conduct initial and ongoing evaluations of the strengths and needs of individuals served.
  • Planning: Collaborate with individuals and team members to develop comprehensive plans that outline their strengths, support needs, and goals.
  • Referral Coordination: Locate services and supports and connect individuals with identified supports to meet their individualized needs and help them achieve their goals.
  • Monitoring & Support: Regularly monitor individual progress and provide ongoing support, guidance, and encouragement to ensure that support plans are being adhered to.
  • Documentation: Maintain accurate and up-to-date records of individual progress, support plans, and any significant changes or developments.
  • Education & Support: Provide education and support to families of individuals, helping them understand the service system and equip them with the necessary tools and strategies to support their loved ones effectively.
Requirements
  • A bachelor's degree or equivalent experience in sociology, social work, psychology, gerontology, criminal justice, or other related fields.
  • 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or other related fields, combined with two years of professional experience in developmental disabilities, special education, mental health, counseling psychology, school psychology, social work, or health and rehabilitative services.
