It is the foundation of The Memorial Experience.Summary:The Patient Navigator is responsible for guiding patients through the complexities of the healthcare system. Working collaboratively with physicians, staff and other health care professionals to provide care coordination across the health care continuum for all assigned patients, the Patient Navigator ensures that safety, best practice, and high quality standards of care are maintained across the continuum. This role provides support and resources to patients, including connecting patients to available services, navigates insurance authorizations, coordinates care between providers, and works with healthcare providers to ensure that patients are receiving the best possible care.Responsibilities:Works closely with physicians and the multidisciplinary care management teams. Facilitates care coordination, and documents efforts. Extract data from medical record sources to gather information and perform outreach. Communicates details to the appropriate team, providers, and/or supervisors.Collect and manage all relevant clinical data (intake forms, photographs, patient health history, labs, images, etc.) prior to consultations/surgeries or procedures. May assist with obtaining authorizations, scheduling appointments and coordinate care between providers, where appropriate, connecting patients to available services, such as primary care providers, specialists, and other community health resources.Assisted patients in coordinating care with other medical professionals. Coordinates patient care plans, including scheduling appointments, following up with providers, and providing/obtaining referrals; helps patients overcome obstacles that they may encounter while pursuing treatment.
