Responsibilities
  • Conduct in-person and remote functional assessments for long-term care eligibility
  • Develop and implement personalized service plans for clients
  • Educate individuals and families about long-term care options and program benefits
  • Ensure accurate data entry into WA Cares Fund systems
  • Make referrals to community resources and Adult Protective Services as needed
Qualifications
  • Bachelor's degree in social services, human services, behavioral sciences, or an allied field
  • Completion of formal training course sponsored by the division within eighteen months of appointment
  • Experience in long-term care, social work, or public health is highly valued
  • Ability to work independently and collaboratively across teams
  • Six years of professional/practical experience performing functions equivalent to a Social Service Specialist 1 may substitute education
Preferred Qualifications
  • Twelve months as a Social Service Specialist 1 and completion of the agency's training program
  • Master's degree in relevant fields with one year of equivalent experience
  • Experience in developing service plans for long-term care clients
  • Strong analytical and interpersonal skills
