We are seeking a skilled Justice Reentry Specialist to join our team.
About the Role:- Our Justice Reentry Specialist will work with adults experiencing severe mental illness, providing person-centered support and connecting them to vital resources.
- The ideal candidate will coordinate care, deliver a warm hand-off to providers, and provide de-escalation techniques to support successful engagement and follow-up care.
- Direct or indirect skill development will also be provided, including information about mental illness, wellness, and recovery.
The successful candidate will be humble, open to coaching and teaching, flexible, able to work alone and with a team, open-minded, and willing to advocate.
Key Responsibilities:- Engagement and Support: Engage with adults experiencing severe mental illness using person-centered approaches, nonjudgmental support, and motivational interviewing to assess re-entry needs and support goal development and maintenance.
- Care Coordination: Coordinate care and deliver a warm hand-off to providers that the individual is being referred to ensure re-entry success and continuity of care while in the community.
- De-Escalation Techniques: Provide de-escalation as needed to support successful engagement and follow-up care.
- Skill Development: Provide direct or indirect skill development, including information about mental illness, wellness, and recovery.
The ability to communicate effectively, develop positive, creative, and programmatic goals, and navigate network drives and basic hardware use and troubleshooting are essential.
