As a skilled Hospice RN On Call, you will be responsible for providing high-quality patient care and support to patients and families in need.
Key Responsibilities:- Perform thorough nursing assessments on new admissions to ensure accurate patient care plans.
- Develop and maintain comprehensive patient care progress records that meet regulatory requirements.
- Collaborate with the clinical manager, physician, and care team to discuss patient progress and make informed decisions.
- Provide education and support to families on patient condition, needs, and care, empowering them to take an active role in their loved one's well-being.
In addition to these responsibilities, you will also be expected to perform on-call services, making referrals to other hospice care disciplines as needed, and completing other duties assigned by management.
Required Qualifications:- A valid RN license specific to the state(s) you are assigned to work.
- No less than one year of nursing experience.
- Current CPR certification.
- A valid driver's license, reliable transportation, and liability insurance.
