Telephonic Case Manager RN

We are seeking a compassionate and dedicated Registered Nurse to join our team as a Telephonic Case Manager. In this role, you will provide comprehensive case management services for patients with multiple or complex conditions.

The ideal candidate will have 5+ years of clinical RN experience in direct patient care and knowledge of case management practices and patient-centered care concepts. They will also be proficient in Microsoft Office, mobile technologies, and navigating multiple applications.

About the Role:
  • Conduct comprehensive assessments with patients and analyze assessment findings to identify and prioritize clinical, psychosocial, and behavioral concerns and potential gaps in care.
  • Develop and document case management care plans in direct collaboration with patients, their families or significant others, primary physicians, and other healthcare providers.
  • Celebrate successes and address challenges throughout the care planning process, ensuring optimal health outcomes for patients.
Key Responsibilities:
  • Document identified issues, prioritized and individualized goals (long & short term), evidence-based interventions, collaborative approaches and resources, anticipated time frames, and barriers to achieving goals in the care plan.
  • Coordinate and implement the activities specified in the care plan to provide optimal benefits coverage as well as promote continuity of care and integration of services for patients across care transitions.
  • Maintain CEUs as required by applicable State Board(s) of Nursing and required certifications.
