Job Overview

We are seeking a seasoned Clinician to lead our team of dedicated healthcare professionals in delivering exceptional client care. As a vital member of our interdisciplinary team, you will be responsible for managing a caseload of clients, developing personalized treatment plans, and coordinating care with external providers.

This role includes field days for staff supervision and oversight of patient care. Administrative tasks can generally be completed off-site unless office presence is needed for operational reasons.

Key Responsibilities:
  • Supervision and Support: Supervise and support direct care staff to ensure compliance with policies and regulations.
  • Caseload Management: Manage a caseload of clients, develop personalized treatment plans, and coordinate care with external providers.
  • Comprehensive Assessments: Conduct comprehensive assessments and supervisory visits, documenting care plans in line with agency standards.
  • Training and Onboarding: Lead orientation, training, and competency verification for new hires.
  • Liaison and Communication: Act as a liaison among staff, community, and healthcare providers to ensure seamless communication.
  • In-Field Supervision: Provide in-field supervision and real-time guidance to clinicians.
  • Quality Assurance: Participate in quality assurance, performance improvement, and staff development initiatives.
  • On-Call Operations: Support on-call operations, ensuring timely follow-up and accurate documentation.
Requirements:
  • A Registered Nurse degree from an accredited school of nursing.
  • Prior home health or pediatric experience is preferred.
