We are seeking a professional to provide essential services and support individuals, families, and treatment processes.

Key Responsibilities:
  • Coordinate service activities and develop self-identified goals for individuals served.
  • Develop Individual Service Plans (ISPs) and monitor progress through regular reviews.
  • Conduct face-to-face visits and phone contact with individuals, families, and/or other pertinent persons involved in the individual's life.
  • Presentation of available treatment services for use by individuals and/or family members.
  • Crisis intervention support through participation in a rotational on-call system.

Requirements:
  • Bachelor's degree in Human Services or related field.
  • One year of experience in human services preferred.
  • Self-motivated and detail-oriented individual with ability to multi-task.
  • Ability to handle crisis situations and exercise sound judgment.
  • Adaptable and able to problem-solve effectively.
