Career as a Nurse Case Manager with Astrana Health
Astrana Health is looking for an LVN or RN to manage case management duties. You will work with pediatrics and/or adult populations depending on business need.
Key Responsibilities:- Comply with departmental policies and procedures
- Ensure 95 - 100% compliance with utilization management health plan delegation standards for inpatient case management
- Utilize Milliman Care Guidelines (MCG) criteria for appropriateness of transition to care by level of skilled needs and acute rehab for post discharge plan
- Collaborate with healthcare professionals to establish discharge plans at time of admission
- Coordinate required initial and concurrent reviews based on medical records received and daily accurate reporting of all inpatient admissions
- Review medical records and other documentation to determine medical necessity, appropriateness and level of care
- Assess, plan, implement, coordinate, monitor and evaluate services to reduce hospital readmission
- Attend and coordinate daily case management meetings as necessary
- Communicate outcomes to providers or members as appropriate
- Provide exceptional customer service through attending to provider calls
- Maintain relationships with health plans, hospitals and medical directors to resolve any issues that may arise
Report activities and problems occurring throughout the day to management.
