Job Overview:

This position is responsible for coordinating and managing case management processes to ensure seamless transitions for patients.

About the Role:
  • Assist Case Managers, Specialists, Supervisors & Managers in assigned areas of responsibility.
  • Provide and coordinate information with outside agencies.
Key Responsibilities:
  • Comply with case management policies and procedures.
  • Support Case Managers on day-to-day activities.
  • Sort, stamp, and distribute incoming faxes.
  • Create authorization/tracking numbers for all discharge planning admissions.
  • Obtain in-patient discharge orders, clinical documents, and follow-up discharge plan dates.
  • Communicate with Hospitals, SNF, Acute Rehab & other admitting facilities on status/updated discharge plans.
  • Provide authorizations for services requested on discharge (i.e., DME, Home Health, others).
  • Update authorization notes to include the status of tracking numbers.
  • Notify admitting facility case management teams & medical group case managers on patient discharge needs status.
  • Investigate problems occurring in the case management department in a timely manner.
  • Respond to department calls and follow up as needed.
  • File and scan hospital records as assigned.
  • Report to CM Lead 3, Supervisor & Manager on daily activities and issues.
  • Attend to provider and interdepartmental calls with exceptional customer service.
  • Demonstrate professional responsibility as a Discharge Planner.
  • Coordinate/manages discharges from In-Patient and SNF units, handling 15-40 discharges daily.
  • Arranges/coordinating discharge plans to Home Health, Hospice, IV, and DME.
  • Follows up with Home Health admissions over weekends.
  • Creates/approves authorizations/cases for Home Health, Hospice, DME, and IV.
  • Reviews TARS 30-70 per day (Treatment Authorization Request) and approves as necessary.
  • Maintains on-call coverage after office hours/weekends when required.
