Lead a Team of Clinicians in Client Care Delivery

As a Registered Nurse Case Manager, you will oversee a team of clinicians to deliver exceptional client care. This involves managing caseloads, collaborating with interdisciplinary teams, training new hires, and coordinating referrals.

You will supervise direct care staff to ensure compliance with policies and regulations. Develop personalized treatment plans and coordinate care with external providers to optimize patient outcomes.

Conduct comprehensive assessments and supervisory visits, documenting care plans according to agency standards. Provide in-field supervision and real-time guidance to clinicians, ensuring they have the necessary support to excel in their roles.

Participate in quality assurance, performance improvement, and staff development initiatives to drive excellence in clinical practice and patient satisfaction.

Requirements
  • Registered Nurse licensure required
  • Degree from an accredited school of nursing
  • Home health or pediatric experience preferred for enhanced skills and knowledge
