Program Coordinator Position Overview

We are seeking a dedicated and compassionate individual to fill a Program Coordinator role. This position offers an opportunity to make a positive impact in the lives of individuals with intellectual, developmental disabilities, and those facing mental health and substance use challenges.

Key Responsibilities:
  • Accept referrals from external sources and confirm eligibility for services.
  • Prepare and submit necessary documentation to coordinate referrals/authorization for services to relevant agencies and organizations.
  • Regularly monitor client insurance eligibility.
  • Serve as a liaison between our organization and various stakeholders, including county offices, managed care companies, government agencies, schools, and other service providers.
  • Ensure services are delivered within authorized timeframes and approved hour limits.
  • Communicate with parents/guardians regarding client progress/concerns through quality-of-service verification calls.
  • Monitor treatment protocols through regular electronic health records checks to ensure compliance with regulated standards.
  • Maintain electronic health records by scanning paper forms/documents regularly to ensure audit readiness.
  • Manage onboarding and offboarding of clients within electronic health records.
  • Perform miscellaneous tasks as assigned by supervisors.
