Our organization is currently seeking a skilled Service Coordinator II to play a vital role in providing support to individuals with developmental disabilities.

The Service Coordinator II will be responsible for coordinating services, advocating for individuals, and providing information and referral to generic services and resources in the community. This includes participating in Interdisciplinary Team (IDT) staffings, attending meetings in the community as needed, and coordinating and securing TCRC funded services such as behavior services, training for independent living, and durable medical equipment and specialized services.

Key Responsibilities:
  • Maintain regular contact with individuals and their families to ensure timely completion of Individual Family Service Plans (IFSP) and Individual Program Plans (IPP).
  • Provide information and referral to generic services and resources in the community, including schools, recreational options, local support groups, and specialist physicians/specialists.

This position requires a Bachelor's degree in a Human Services or related field and a minimum of 2 years of social and/or mental health experience. A valid California driver's license and transportation or acceptable substitute are also required for this position. Experience working with individuals with developmental disabilities is preferred but not required.
