We're seeking a skilled rehabilitation specialist to join our team. As a key member, you'll play a vital role in helping patients recover from injuries or illnesses and regain their independence.
About the Role:
- Title: Rehabilitation Specialist
- Location/Work Environment: In facility
- Reporting Structure: Reporting to Director of Rehabilitation
You'll work closely with patients to assess their needs, develop personalized recovery plans, and deliver tailored physical therapy sessions to restore and enhance their physical abilities. Your collaborative approach will ensure seamless communication with healthcare teams and facilitate referrals when necessary.
Key Responsibilities:
- Conduct thorough patient assessments to identify areas for improvement
- Develop and implement comprehensive recovery plans aligned with patient goals
- Deliver high-quality physical therapy sessions to promote optimal outcomes
- Collaborate with multidisciplinary teams to ensure coordinated care
- Maintain accurate records and communicate effectively with patients and healthcare professionals
Requirements:
- A valid Physical Therapy license in the state(s) of practice or proof of eligibility
- A Master's degree in Physical Therapy or a related field
- At least 5 years of progressively responsible PT experience
- Ability to work in a fast-paced environment and adapt to changing situations
