Home Health Nurse Position

We are seeking a highly skilled Home Health Nurse to join our team.

  • This position involves coordinating and overseeing all aspects of patients' home care, including developing and implementing care plans in collaboration with patient care teams.

Responsibilities include:

  • Educating families on patient condition, needs, and care
  • Performing head-to-toe physical assessments and checking vital signs
  • Managing medications, giving injections and IV therapy, and administering catheters
  • Completing documentation timely and accurately and supervising LPNs and HHAs
  • Regularly communicating patient progress to the clinical manager, physician, and care team

Required qualifications for this role include:

  • A minimum of one year of clinical experience as a registered nurse (RN)
  • Current RN license, specific to the state(s) you are assigned to work
  • A minimum of six months of home health experience and current CPR certification
  • A valid driver's license, reliable transportation, and liability insurance
