Job Title: Rehabilitation Therapist

We are seeking a skilled Rehabilitation Therapist to join our team. As a Rehabilitation Therapist, you will play a vital role in helping patients recover from injuries or illnesses and regain their mobility and independence.

Responsibilities:
  • Conduct thorough assessments of patients' physical abilities and create personalized treatment plans to address their specific needs.
  • Develop and implement interventions to help patients improve their strength, flexibility, and range of motion.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
  • Provide ongoing support and guidance to patients and their families to promote continued progress and empowerment.
Qualifications:
  • A graduate degree in Physical Therapy (DPT) from an accredited program.
  • Current licensure to practice as a Physical Therapist in the state where you will be working.
  • At least 1 year of experience in home health physical therapy, with a strong focus on patient-centered care and outcomes-based practice.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and interdisciplinary teams.
