Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our Home Health team. As a Registered Nurse, you will have the opportunity to provide direct patient care within the comfort of their own homes, making a meaningful difference in the lives of our patients.

Responsibilities

  • Conduct comprehensive assessments and complete OASIS timely to ensure accurate and complete patient care.
  • Draft and implement care plans, reconcile patient medications, and ensure orders for care are accurate and complete.
  • Prepare accurate, orderly, and timely documentation to support patient care.
  • Coordinate care with patients and provide proper reports to Case Managers.
  • Provide health care instructions to patients and families as appropriate per assessment and plan.

Requirements

  • An active RN license in the state in which practicing or New Grad with eligibility to take NCLEX.
